Position: Registered Nurse - Caregiver support Program Coordinator

Summary

The Caregiver Support Program (CSP) Coordinator Registered Nurse (RN) executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management. The RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The RN provides a state‑of‑the‑art continuum of services, education, and resources to caregivers of Veterans with complex needs.

Qualifications
  • English language proficiency (basic written and spoken English required).
  • Graduate of an accredited nursing school (ACEN or CCNE) or equivalent foreign education with valid equivalency assessment.
  • Or completion of a master’s level bridge program in nursing, with a state license to be obtained before graduation.
  • Upon license achievement, the individual may be appointed on a temporary basis and later converted to permanent appointment.
  • Graduate Nurse Technician (GNT) Exception: candidates may be appointed at the entry level on a 120‑day temporary appointment while pursuing licensure (may be extended up to two years).
  • Grandfathering Provision: current VHA employees in the 0610 series performing the duties described are considered to have met all qualification requirements as of 1/29/2024.
  • Grade determination considers scope, education, and dimension criteria (Practice – Veteran/Patient Driven Care – Leadership – Professional Development – Evidence‑Based Practice/Research).
  • Applicable qualifications for Nurse I (Levels I‑III), Nurse II, and Nurse III include combinations of ADN, BSN, MSN, and doctorates as outlined in the qualification standard (link: https://(Use the "Apply for this Job" box below). Standards/).
  • Physical requirements:

    moderate lifting (15–44 lbs), moderate carrying (15–44 lbs), use of fingers, both hands required, operation of crane, truck, tractor, or motor vehicle, valid driver’s license.
  • Environmental factors: outside and inside, slippery or uneven walking surfaces, working closely with others, working alone.
Duties
  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
  • Provides a state‑of‑the‑art continuum of services, education, and resources to caregivers of Veterans with complex needs.
  • Coordinating care for the Caregiver Support Program, which includes PCAFC and PGCSS within the VHA.
  • Assess, plan, implement, and evaluate care based on age‑specific components.
  • Assumes responsibility for the coordination of care focused on patient and caregiver education, self‑management, and customer satisfaction throughout the continuum of care.
  • Displays knowledge of eligibility criteria for PCAFC and/or PGCSS, day‑to‑day operation of the program, and all reporting requirements within the assigned VISN.
  • Follows procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
  • Works with interprofessional team members at facility and VISN levels to ensure support to caregivers is delivered objectively with strong, consistent documentation and communication in a direct and coordinated manner.
  • Advocates for holistic care through patient/caregiver education and provider interaction, implementing an educational plan to meet the patient care needs.
  • Fosters a safe and supportive environment conducive to the professional development of health care professionals.
