Functions or scope of assigned duties include but are not limited to the following:

• Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
• Provides a state-of-the-art continuum of services, education, and resources to caregivers of veterans with complex needs.
• The caregiver support program (CSP) includes the program of comprehensive assistance for family caregivers (PCAFC) and the program of general caregiver support services (PGCSS) within the veterans health administration (VHA).
• Assesses, plans, implements, and evaluates care based on age-specific components.
• Assumes responsibility for the coordination of care focused on patient and caregiver education, self-management, and customer satisfaction throughout the continuum of care.
• Displays knowledge of eligibility criteria for the PCAFC and/or PGCSS, day-to-day operation of the program, and all reporting requirements within the assigned veterans integrated service network (VISN).
• Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team.
• Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.
• Works with interprofessional team members at facility and VISN levels to ensure support to caregivers is delivered objectively with strong, consistent documentation and communication in a direct and coordinated manner.
• Advocates for holistic care through patient/caregiver education and provider interaction and implements an educational plan to meet the patient care needs.
• Fosters a safe and supportive environment conducive to the professional development of health care professionals.
