Hospice Registered Nurse (RN) – Case Management
Seeking an experienced Hospice RN with at least 1 year of current hospice case management experience. The RN will manage a caseload of approximately 15 patients and provide comprehensive hospice nursing services based on patient needs and acuity.
Manage a caseload of approximately 15 patients.
Complete 4–5 patient visits per day, depending on service area and patient acuity.
Complete charting the same day as visits.
Provide all levels of hospice nursing care, including:
- Routine hospice care.
- General Inpatient (GIP).
- Continuous care.
- Respite care.
Perform admissions, Symptom/Intensive Assessment (SIA), and death visits.
Cover a service radius of approximately 60 miles.
Mileage reimbursed at the applicable IRS standard mileage rate.
Clinicians may float to Oregon sites.
Minimum 1 year of current Hospice Case Management experience – REQUIRED.
Active Washington RN or Compact license – REQUIRED.
Active American Heart Association BLS – REQUIRED.
Valid driver's license and reliable vehicle – REQUIRED.
Valid auto insurance – REQUIRED.
Clean background check in Washington and Oregon.
Successful drug screening.
Influenza, TB, and COVID vaccination records/documentation.
Oregon RN license strongly encouraged.
Oregon RN licensure application is required upon accepting the contract if not already licensed.
Updated resume/work history; gaps of 30+ days must be explained.
Hospice-specific Skills Checklist.
2 supervisory references from the last 2 years of employment.
Active Washington or Compact RN license.
American Heart Association BLS certification.
Washington RN License or Compact License – Required.
Oregon RN License – Strongly Encouraged.
BLS – American Heart Association – Required.
