Travel PTA - Home Health

American Traveler is seeking a Home Health PTA with HCHB experience and 1+ years of recent geriatric care experience for a day shift assignment.

• Current BLS certification required
• Minimum of 1 year of recent relevant experience required
• Proficiency in HCHB (Homecare Homebase) required
• OASIS experience required for staff completing Start of Care (SOC)
• Active driver's license and personal transportation required
• License required if applicable
• License must be in-hand prior to consideration

• Home Health setting with a caseload of 5-7 patients per day
• Patient population consists of geriatric cases
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
• Territory covers up to a 60-mile radius from the hub office, with flexibility required
• Uses HCHB (Homecare Homebase) EMR for documentation
• No on-call requirement, though on-call rotation may be discussed during interview
• Responsibilities include home exercise programs, patient and family education, and documentation
• Weekly productivity requirement of 30 points
• Local candidates accepted at the travel rate
• First-time travelers will not be considered
• Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, though each traveler is assigned an individual territory and must have their own transportation
• Scrubs required in any color; jeans are not permitted
• iPad provided for work use; personal cell phone not required
• Mileage reimbursed at the IRS standard rate
• Orientation held on Mondays, including completion of billable modules
• RTO must be submitted at the time of application
