Home Health Registered Nurse (RN)

We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) to provide skilled nursing services to patients in Groton, Connecticut and surrounding communities. This position is responsible for delivering high-quality patient care in the home setting, completing assessments, documenting visits, coordinating care with the interdisciplinary team, and ensuring positive patient outcomes. The ideal candidate is clinically skilled, organized, and comfortable working independently while managing a home health caseload.

Groton, CT and surrounding towns

Schedule:

Monday through Friday, approximately 9:00 AM to 5:00 PM

Hours:

8 hours per day

Pay Rate: $51.00 - $53.00 per hour

On-Call/Weekend Requirement:
None

Mileage Reimbursement:
Yes, reimbursed between patient visits

Key Responsibilities
  • Provide skilled nursing care to patients in their homes in accordance with physician orders and established care plans.
  • Complete comprehensive patient assessments and ongoing evaluations of patient health status.
  • Perform skilled nursing visits and interventions based on individual patient needs.
  • Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR) system.
  • Participate in case conferences and collaborate with the interdisciplinary care team to ensure coordinated patient care.
  • Communicate effectively with physicians, caregivers, and other healthcare professionals regarding patient progress and changes in condition.
  • Educate patients and family members on disease processes, medications, treatment plans, and self-management strategies.
  • Monitor patient outcomes and recommend adjustments to care plans as appropriate.
  • Adhere to agency policies, clinical standards, and regulatory requirements.
  • Support quality improvement initiatives and patient satisfaction goals.
Required Qualifications
  • Active Registered Nurse (RN) license in the state of Connecticut.
  • Minimum Associate Degree in Nursing (ADN) required.
  • Strong clinical assessment and patient care skills.
  • Excellent written and verbal communication abilities.
  • Ability to work independently and effectively manage a field-based caseload.
  • Valid driver's license, reliable transportation, and proof of automobile insurance.
Preferred Qualifications
  • Previous home health experience.
  • Experience using electronic medical record (EMR) systems.
  • Strong organizational and time management skills.
  • Excellent interdisciplinary collaboration and care coordination abilities.
  • Patient-focused approach with strong critical thinking and problem-solving skills.

  • Business casual attire or scrubs permitted.
