Salary Range: $45.00 To $55.00 Annually
Day Shift / 40hrs/wk
This position, under the direction of the Associate Executive Director of Home Health, is accountable for ongoing planning, organization, direction, evaluation and safe delivery of high-quality care to adult and geriatric, clients in accordance with agency’s Policies and Procedures, Mission, State and Federal Regulations.
Essential Duties and Responsibilities:
Accepts appropriate referrals to the agency and assigns qualified professional staff to meet the needs of the adult and geriatric client. Assigns work to staff, provides direction and reviews work.
Assists in the supervision all assigned nursing staff in the delivery of nursing services to Adult and geriatric clients and families.
In conjunction with clinical staff and manager, prepares on-call schedules, plans and organizes daily assignments including the review and assignment of new patient referrals, facilitates huddle calls and staff meetings.
Conducts adult and geriatric patient clinical record reviews to assess the appropriateness of care plan, the utilization of services and to ensure complete documentation of needed services.
Assumes an active role in the professional development of assigned staff, including participation in orientation, ongoing skill development and other mentoring activities. Responsible for maintaining an office environment that fosters the professional growth and development of assigned staff.
In conjunction with Human Resources, participates in staff recruitment, selection and discipline/termination of employment of staff not performing according to agency standards (requires consultation with administration).
Performs periodic home visits with staff for the purpose of role modeling, problem solving, supervising and evaluating performance/quality of care. Mentors new staff nurses.
Demonstrates timely and accurate computer entry of patient/job information to facilitate the development of the OASIS data set, orders for service and other required patient/employment documents and reports.
Identifies, develops and provides in-service/continuing education for the staff.
Updates Management Information System data consistently.
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
Completes annual education & medical requirements.
Manages and oversees a patient caseload including visiting patients per MD orders.
Performs other duties as required.
Minimum Qualifications:
Education:
BSN degree; or certification by the American Nurses’ Association as a community health nurse or completion of at least six (6) credits within two (2) years in community health nursing theory or six (6) credits in health care management from an accredited college or university program or school ofExperience:
Minimum of two (2) years’ experience as an RN Case Manager (field nurse in homecare)Certificates, Licenses, Registrations:
Current Connecticut RN License
