RN) Registered Nurse - Home Health | Wallingford, Connecticut

Schedule:

• 13-week contract with potential for extension
• Full-time, Monday through Friday
• On-call duties may be required
• Structured daily visit expectations

Responsibilities:

• Deliver in-home nursing care, skilled interventions, and comprehensive assessments
• Accurately complete OASIS documentation and maintain timely records
• Coordinate with physicians and interdisciplinary teams to implement individualized care plans
• Perform wound care, manage medications, educate patients and families, and monitor disease progression
• Communicate patient updates and any changes in condition promptly to the clinical team

Qualifications:

• Active Registered Nurse license in good standing
• At least one year of recent RN experience in a home health setting
• Proven OASIS documentation skills
• Familiarity with Home Care Home Base (highly preferred)
• Reliable transportation and valid drivers license
  
  Strong organizational skills to manage full-time caseload

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
