Walnut Creek, CA Compensation: $46.00-$48.00 per hour (Based on experience)
Role Overview Shadelands Post Acute is looking for a passionate and skilled Certified Occupational Therapist Assistant (COTA) to join our dynamic in-house therapy team. In this role, you will work under the direction of our Occupational Therapists to help residents regain their independence and improve their quality of life. Key Responsibilities
* Patient Treatment:
Implement therapeutic treatments as directed by the Occupational Therapist and the physician’s care plan to help patients master activities of daily living.
* Clinical Documentation:
Maintain meticulous records, including daily treatment notes and weekly progress reports, in full compliance with the OT Board State Practice Act and payer requirements.
* Interdisciplinary
Collaboration:
Communicate effectively with supervisors and the broader healthcare team regarding patient progress, goals, and discharge plans.
* Education & Training:
Instruct nursing staff and families in maintenance programs and caregiver techniques; assist in training Restorative Aides.
* Discharge Planning:
Actively participate in the discharge process to ensure patients have a safe and successful transition home.
* Environment Stewardship:
Assist in the cleaning and maintenance of the treatment area to ensure a safe, high-functioning therapy environment.

Qualifications
* A valid, current Occupational Therapist Assistant (COTA) License in the state of California.
* Strong communication skills and the ability to relate positively to residents, families, and colleagues.
