Occupational Therapist – UHC Staffing Occupational Therapist

Location: Walnut Creek, CA

Contract Type: Per-Diem

Shift Type: PRN / Per Diem — variable schedule (as-needed; shifts based on facility staffing needs)

Candidate Pay Rate: $61.6 per hour

• Perform thorough patient evaluations to assess functional performance, self-care skills, and rehabilitation needs
• Develop and implement personalized treatment plans tied to meaningful patient goals
• Deliver therapeutic activities, exercises, and adaptive strategies to maximize independence
• Monitor and document patient progress while ensuring compliance with state and federal guidelines
• Educate patients and families on therapy goals, adaptive equipment, and home exercise programs
• Collaborate proactively with PTs, nurses, physicians, speech therapists and other team members to coordinate care

• Active and current Occupational Therapist (OT) license in California (required)
• Graduate of an accredited Occupational Therapy program (OTD or equivalent)
• Previous experience working in a Skilled Nursing Facility (SNF) is required
• Strong clinical knowledge with a patient-centered approach
• Excellent communication, organization, and teamwork skills

• Doctorate of Occupational Therapy (OTD) or equivalent from an accredited Occupational Therapy program

• Patient evaluation and functional assessments

• Therapeutic activities and adaptive strategies
• Patient and family education for home programs
