We are seeking an experienced and passionate
Home Health
Clinical Manager to join our growing team. In this critical leadership role, you will oversee the clinical team, ensure exceptional patient care, and help shape the future of our clinical practices. Your leadership will directly impact patient outcomes, staff development, and overall operational excellence.

• Lead and support a multi-disciplinary clinical team
• Oversee admissions, care plans, and physician orders
• Ensure compliance with Medicare, state, and federal regulations
• Supervise clinical operations to maintain quality standards
• Conduct patient visits as needed for care oversight
• Manage survey readiness and regulatory compliance
• Recruit, train, and support new clinical staff
• Monitor performance, address re-hospitalizations, and manage productivity
• Coordinate with case managers and marketing for referral development
• Serve as a key liaison between leadership and field staff

You’ll work with a team that is passionate about delivering excellence and treating every patient and staff member with dignity and respect.

• Minimum 1 year of Home Health experience
• At least 1 year of clinical management experience
• 2–3 years of RN nursing experience (preferably in leadership roles)
• Knowledge of Federal, State and Local home healthcare regulations for best practices
• Strong leadership, communication, and problem-solving skills
• Demonstrated success in staff coordination, training, and performance management
