Home Health Clinical Manager
Salary: $145,000–$190,000 (DOE)
Sign-On Bonus: $15,000
Location:
Walnut Creek, CA
- Lead and support a multi-disciplinary clinical team
- Oversee admissions, care plans, and physician orders
- Ensure compliance with Medicare, state, and federal regulations
- Supervise clinical operations to maintain quality standards
- Conduct patient visits as needed for care oversight
- Manage survey readiness and regulatory compliance
- Recruit, train, and support new clinical staff
- Monitor performance, address re-hospitalizations, and manage productivity
- Coordinate with case managers and marketing for referral development
- Serve as a key liaison between leadership and field staff

- Active California RN License (required)
- Minimum 1 year of Home Health experience
- At least 1 year of clinical management experience
- 2–3 years of RN nursing experience (preferably in leadership roles)
- Knowledge of Federal, State and Local home healthcare regulations for best practices
- Strong leadership, communication, and problem-solving skills
- Demonstrated success in staff coordination, training, and performance management
