Registered Nurse (RN) | Case Manager

Shift Information: Days - 5 days x 8 hours

About the Position
Coordinate safe and efficient patient care in an acute care hospital setting, focusing on discharge planning and transitions of care.
Develop individualized care plans in collaboration with the interdisciplinary team and advocate for patients and families.
Utilize the hospital EMR (EPIC preferred) to document, track progress, and communicate with physicians, nursing, and support staff.
Requirements
Previous travel RN experience
EPIC EMR experience
Active California RN license
Minimum 2+ years of recent acute care hospital experience in Case Management/Utilization Review/Discharge Planning
Strong clinical assessment, critical thinking, and care coordination skills
