Required Qualifications- 2 years of Case Management, Acute Care Experience, direct patient care delivery.
- Experience in utilization review/Case mgt, and discharge planning; care coordination across the continuum of care in collaboration with the physician and other members of the health care team.
- Discharge Planning, explanation of benefits, Kaiser Health Connect, Care Port.
- ICU/ED Experience
- Health Connect/Epic experience
