Key Responsibilities
** Clinical Care
*** Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs
* Deliver compassionate, resident-centered care focused on comfort, dignity, and safety
** Assessment & Monitoring
*** Observe, assess, and document changes in resident condition
* Communicate concerns promptly to the RN Supervisor and healthcare providers
** Care Planning
*** Assist with the development and implementation of individualized care plans
* Support resident recovery goals and quality-of-life initiatives
** Leadership & Team Support
*** Supervise and support Certified Nursing Assistants (CNAs)
* Collaborate with interdisciplinary team members to ensure continuity of care
** Documentation & Compliance
*** Maintain accurate clinical documentation and medical records
* Ensure compliance with all state, federal, and facility regulations and standards

Qualifications Current and active California LVN license required   Current BLS/CPR certification required   Previous skilled nursing or post-acute care experience preferred   Strong clinical judgment and communication skills   Compassionate, dependable, and team-oriented approach to resident care
