In this leadership role, reporting to the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation, ensuring support is easily accessible in a timely fashion; participate in the identification, development, design, and implementation of nursing educational programs;

and determine success measures and evaluate program effectiveness to promote ongoing staff development and patient satisfaction. You will meet regularly with our customers to maintain relationships and provide updates such as readmission reports.

• Field oversight of clinicians who provide care to patients in their homes.
• Handle incoming calls promptly and follow up to ensure appropriate actions.
• Coordinate training and educational programs for field nurses.
• Assess staff learning needs, formulate plans, perform administrative duties, and serve as a role model.
• Act as first point of contact for field clinicians to assist with patient care escalations.
• Coordinate at least bi‑weekly case conferences to discuss patient progress and revise care plans.
• Review and evaluate each case, provide guidance to clinicians, and be available during operating hours.
• Review patient clinical diagnoses, medications, procedures, and clinical course.
• Assist clinicians in establishing therapeutic goals, setting priorities, and developing care plans.

• Two years of management or supervisory experience in the past five years within a health‑care setting, preferably in home health.
• Graduate of an accredited nursing school;
  Bachelor’s Degree preferred.
• Current California Registered Nurse license (RN).
• Current Basic Life Support (BLS) card.
• Valid California driver’s license with a clean driving record.

Annual salary ranging from $143,000 to $163,000, plus a generous annual bonus of $40,000 paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers comprehensive health benefits and a 401(k) plan with up to a 4% match.
