Registered Nurse (RN) | Case Manager
Location: Walnut Creek, CA
Agency:
Pay: $2,400 to $2,700 per week
Shift Information: Days - 5 days x 8 hours
Contract Duration: 13 Weeks
Start Date: ASAP
- Coordinate safe and efficient patient care in an acute care hospital setting, focusing on discharge planning and transitions of care.
- Develop individualized care plans in collaboration with the interdisciplinary team and advocate for patients and families.
- Utilize the hospital EMR (EPIC preferred) to document, track progress, and communicate with physicians, nursing, and support staff.
- Previous travel RN experience
- EPIC EMR experience
- Active California RN license
- Minimum 2+ years of recent acute care hospital experience in Case Management/Utilization Review/Discharge Planning
- Strong clinical assessment, critical thinking, and care coordination skills
