In this leadership role reporting the Director of Nursing, you will be responsible for ensuring that patient care is coordinated and managed appropriately by a team of Registered Nurse (RN) Case Managers and LVNs. You will serve as the first point of escalation, supporting timely access to care, and participate in the development, design, and implementation of nursing educational programs to promote clinical excellence and improve patient outcomes.
HybridWork Model
- Office: 2 days per week in Walnut Creek for administrative and team management duties.
- Field: 3 days per week (average 15+ visits per week) conducting supervisory visits and offering direct support to RN Case Managers.
- Oversee clinicians who provide care in patients’ homes.
- Handle incoming calls timely and follow up to meet internal and external customer needs.
- Coordinate training and educational programs for nurses to improve clinical outcomes and competencies.
- Assess staff learning needs and formulate plans and goals for evaluation periods.
- Act as the first point of contact for field clinicians to assist in patient care escalations.
- Coordinate at least bi‑weekly case conferences with the clinical team to discuss patient progress and revise care plans.
- Review and evaluate each case by reviewing services, conferences, record review, and guiding clinicians to improve performance.
- Review patient’s clinical diagnosis, medications, procedures, and clinical course.
- Assist clinicians in establishing immediate and long‑term therapeutic goals, setting priorities, and developing a plan of care.

- Two years of management or supervisory experience within the past five years in a healthcare setting, preferably in home health.
- Graduate of an accredited school of nursing; bachelor’s degree preferred.
- Current California Registered Nurse license (RN).
- Current Basic Life Support (BLS) card.
- Valid California driver’s license with acceptable driving record.
- All licenses and certifications must be current at time of hire and sustained throughout employment.
