OverviewWe are looking for positive and personable individuals to assist nursing staff in providing care to people with intellectual and developmental disabilities, mental health, and substance use challenges. The role is based in our home healthcare team and involves direct patient support and administrative duties.
Responsibilities- Report changes in condition to nurse immediately
- Work with nurse and administration to keep persons safe and healthy
- Record entries on flow sheets, notes, charts accurately and timely
- Measure vital signs and notify nurse of changes
- Assist persons with dressing, hygiene, toileting
- Administer medications and feedings via gastrostomy tube as ordered
- Monitor oxygen administration and report findings
- Check and monitor oxygen saturation levels
- Perform colostomy care
- Maintain confidentiality of all patient care information
- Provide care in privacy
- Fax orders to pharmacy, complete labs to doctors, assist with OTC stock and supplies, schedule appointments and other office duties
- Participate in the team by receiving nursing reports, assisting in transport of persons to appointments, activities, and out‑of‑center appointments as necessary
- Assist with lifting, turning, moving, positioning, and transporting persons to/from beds, chairs, bathtubs, wheelchairs, lifts, etc.
- Lift up to 20 lbs routinely; lift up to 40 lbs when required
- Pull minimum of 25 lbs
- Squat, kneel, crawl, crouch, climb, stoop
- Use hands to finger, handle or feel objects, tools, or controls
- Stand and walk regularly
- Demonstrate proficiency in CPR from the floor level, working on hands, knees, bending, standing and lifting
- Vision: close, distance and peripheral vision; ability to talk and hear
- High school diploma or GED
- Licensed Certified Nursing Assistant (preferred but not required)
- Valid driver’s license and personal vehicle insurance
- Maintain proficiency in company‑sponsored training and certifications
- CPR and first aid skills; ongoing proficiency required
