Outstanding Per Diem Registered Nurses needed for field-based positions servicing Alameda and Contra Costa County locations:
Orinda, Moraga, Lafayette, Walnut Creek, Alamo, Danville, San Ramon, Dublin Pleasanton and Livermore

Job Description:

Registered Nurse - Home Health

Responsible for the assessment and plan of care of home health patients in accordance with the RN scope of practice and in compliance with CMS guidelines and regulations for home health care. Responsible for creating the OASIS/start of care documentation, re-visits following start of care, care coordination and managing a team of therapists to ensure patients meet their home health goals, and can be discharge back to the community with emphasis on an excellent patient experience.

Responsibilities
  • Assess and plan the care of home health patients in accordance with RN scope of practice and CMS guidelines.
  • Create OASIS/start of care documentation and perform follow-up visits post-start of care.
  • Coordinate care and manage a team of therapists to help patients achieve home health goals.
  • Support discharge planning back to the community with emphasis on excellent patient experience.
Registered Nurse Requirements
  • 1 year full-time experience in Med/Surg or an acute setting; 1 year minimum home health experience.
  • RN unrestricted license in California, CPR, California Driver’s License, with valid auto insurance.
  • Skilled in IV infusion, phlebotomy, wound care including wound vacs, NGT/GT, teaching and management of all disease processes.
  • Able to manage an independent schedule, supervise LVN’s & CHHA’s, excellent communication skills, integrity and loyalty.
  • Maintain flexibility with the challenges of Bay Area travel from home to home.
  • Experienced in home health software and other electronic communication.
  • Critical thinking, time management and attendance at case conferences/staff meetings.
  • Able to work one weekend per month (optional).

Compensation

  • Estimated Annual Full Time Compensation: $124K to $165K (per-visit pay plus mileage).
