Job Summary

We are seeking passionate and dedicated Physical Therapists to join our team at Stellar Health Group in Walpole, MA. As a Physical Therapist, you will play a vital role in providing high-quality rehabilitation services and helping residents achieve their highest level of mobility and independence. Responsibilities
  • Provide physical therapy services to the facility, promoting excellence in rehabilitation care, interdisciplinary cooperation, and quality patient care

  • Evaluate patients in a timely manner, as required by state, payer source, or facility requirements

  • Screen patients upon admission and/or when requested by the director

  • Develop an appropriate plan of treatment and goals for each patient

  • Provide treatment according to the plan developed after patient evaluation

  • Communicate with other disciplines and interact with staff as necessary based on the treatment plan

  • Provide routine documentation according to Medicare, facility, and other payer source guidelines

  • Participate in family conferences when requested

  • Submit all required documentation and treatment charges in a timely manner, in accordance with policy and procedure

Qualifications
  • Current MA State License as a Physical Therapist

  • Ability to work as part of a dynamic and cooperative team while ensuring workload is distributed equitably across the department

  • Capability to evaluate and assess patients and participate in consultations and teaching

  • Ability to collaborate with residents, staff, families, and other professionals to plan individualized treatment plans and goals

  • Ability to discuss and plan appropriate discharge plans

  • Ability to teach and counsel patients and staff on best practices for recovery
