Job Description

Milestones Day School and Transitions Program is a year-round private special education school located in Waltham, MA and works with students with autism spectrum disorders, learning disabilities, and a variety of attention and/or psychiatric challenges. We service students ranging in age from elementary grades through post high school, and our staff work in multi-disciplinary teams to support students in particular grade cohorts.

We are currently recruiting for a Case Manager/Mentor Teacher to join our Middle School team for the upcoming  School Year.

Active Massachusetts Licensure Moderate Disabilities (PreK-8 or 5-12) OR Severe Disabilities
- All, is REQUIRED

Job Summary:

  • Manage a caseload of students, communicate with families as needed, and support students in having successful outcomes.
  • To provide guidance and support and to role model appropriate teaching practices for teaching team.
  • This role is highly visible and the Case Manager is responsible for being an effective leader and mentor at Milestones.

Detailed Responsibilities:

1. Lesson Plans/Teaching Support

  • Role model best teaching practices for other teachers and provide differentiated instruction to students.
  • Teach lessons in licensed areas as assigned
    - Observe and develop relationships with teachers, offering general suggestions, reviewing IEPS and lesson plans
  • In conjunction with Principal, meet with teachers to outline together the teachers’ goals for the year, including professional development, previous reviews, and Milestones’ goal for the year
  • Review all unit plans and weekly lesson plans prior to execution (as required by Principal)
  • Meet with teaching teams at least once per week to provide feedback, reflection, support, and collaboration
  • Regular classroom presence with the goal of providing instruction, training, and modeling teaching strategies.

2. Liaison Role for Students and Their Teams

  • Regular communication with classroom and therapeutic staff to coordinate appropriate programming/plans for each assigned student
  • Provide direct support to students (i.e. previewing, processing, de-escalating, reviewing progress) to best support their instruction and student engagement.
  • Maintain confidentiality

3. IEPs

  • Assure that the sections for Student Strengths, Current Performance, and Goals/Benchmarks are written for students’ IEPs.
  • Review the PLEP A & B to determine if the student needs additional items listed or to take away items that are not relevant
  • Specific responsibilities with regard to writing IEPs are determined by Principal

4. Case Management

  • Speak with parent(s) by phone at least once per week; and document these phone calls along with the student’s week in a written email once per week (cc family, entire team, and Principal)
  • Talk with team members regularly to maintain a broad understanding of how the student is doing and communicate this to parents
  • Contact student’s family by phone any time there is a problem during the day; communicate the outcome of the conversation with the team
  • Assure that students corrected work samples are sent home each week
  • Share with team and Principal when parents provide concerns or positive feedback

5. Special Academic Events Involving Families

  • Coordinate with leadership staff to help plan these events and act as an ambassador from Milestones to promote customer service
Requirements

Active Massachusetts Licensure Moderate Disabilities (pk-8) or (5-12) OR Severe Disabilities
- All, is REQUIRED

  • Bachelors Degree Required, Masters strongly preferred.
  • Teaching experience encompassing education and/or human services is required.
  • Mentor, Academic Coaching of peers, and/or Leadership experience preferred.
