Job Summary

The Habilitation Coordinator plays a key role in supporting adults with developmental disabilities by designing meaningful, person-centered programming and ensuring the effective daily operation of the Day Habilitation site. This position provides leadership, staff supervision, curriculum development, and advocacy to help individuals build independence, skills, and community connections.

Key Responsibilities Program Planning & Individualized Support
  • Develop, implement, and monitor ISP/DHSP objectives for individuals on assigned caseloads
  • Complete assessments and create intervention strategies with appropriate data collection methods
  • Ensure required daily, monthly, and quarterly documentation is accurate and timely
  • Collaborate with the Interdisciplinary Team and attend all ISP/DHSP meetings
  • Continuously evaluate the effectiveness of goals and support plans
Daily Operations & Site Coordination
  • Organize daily schedules that integrate individual goals, auxiliary services, and therapeutic activities
  • Assign staff resources effectively to support daily programming
  • Coordinate transportation for community-based activities
  • Ensure vehicle safety procedures and inspections are followed
Staff Supervision & Training
  • Assist with interviewing, onboarding, training, and evaluating Habilitation Specialists
  • Conduct bi-weekly supervision meetings focusing on development, training needs, curriculum planning, and individual progress
  • Ensure all staff maintain required certifications and receive ongoing training
  • Collaborate with leadership to plan monthly staff meetings
Advocacy, Rights, & Safety
  • Support a culture of dignity, choice, and self-determination for everyone served
  • Ensure a certified Human Rights Officer is active on-site and provide required monthly Human Rights trainings for individuals and staff
  • Promote safe practices and ensure staff understand and follow emergency procedures, fire safety protocols, and incident reporting
  • Conduct and document monthly fire drills
Program Quality & Enrichment
  • Develop and maintain a dynamic habilitation curriculum
  • Manage materials, equipment, and functional spaces (e.g., art room, fine motor areas)
  • Foster an inclusive, energetic, and relationship-driven environment
  • Explore community opportunities, volunteer sites, and therapeutic activities while monitoring their ongoing value and impact
Qualifications
  • Minimum of two years’ experience working with adults with developmental disabilities; one year in habilitation services preferred
  • Associate degree or equivalent relevant experience
  • Valid driver’s license and acceptable driving record
  • Current First Aid/CPR certification (or ability to obtain)
  • Strong communication, documentation, and problem-solving skills
Physical Requirements
  • Ability to stand, walk, and move for extended periods throughout the day, including stairs. Must be able to physically complete the evacuation plan while supporting Individuals.
  • Frequently lift, carry, push, or pull up to 25 lbs (e.g., files, supplies, groceries)
  • Occasionally lift or move up to 50 lbs (e.g., assisting with transfers, repositioning individuals, crisis prevention, pushing individuals in wheelchairs, using Hoyer lifts)
  • Ability to provide hands-on support for personal care as needed per individual support plans (e.g., showering, toileting, assistance with eating)
  • Ability to perform basic household tasks related to program operations (e.g., meal preparation, laundry, routine household cleaning, snow shoveling)
  • Ability to operate or ride in a vehicle; driving is a requirement for most positions and includes driving agency vans, including wheelchair vans, or personal vehicles
    Ability to secure wheelchairs and assistive devices as needed.
