Job Summary
The Habilitation Specialist provides direct support to individuals with acquired brain injury, intellectual and/or developmental disabilities in WCI’s Day Habilitation Services program. This role focuses on implementing Individual Service Plans (ISP) and Day Habilitation Service Plans (DHSP), supporting independence, and promoting community inclusion.
Essential Functions- Implement DHSP and ISP objectives to help individuals build skills and participate in meaningful activities.
- Assist individuals in making choices and engaging in community-based experiences.
- Document daily progress, incidents, and therapeutic goals accurately.
- Provide physical assistance as needed, including transfers, mobility, hygiene, and eating.
- Respond to behavioral incidents following established plans and guidelines.
- Maintain a safe, clean, and stimulating environment.
- Ensure and safeguard individuals’ dignity, rights, and liberties
- Transport individuals during community activities.
- Attend staff meetings, trainings, and work collaboratively as part of a team.
- Ability to stand, walk, and move for extended periods throughout the day, including stairs. Must be able to physically complete the evacuation plan while supporting Individuals.
- Frequently lift, carry, push, or pull up to 25 lbs (e.g., files, supplies, groceries)
- Occasionally lift or move up to 50 lbs (e.g., assisting with transfers, repositioning individuals, crisis prevention, pushing individuals in wheelchairs, using Hoyer lifts)
- Ability to provide hands‑on support for personal care as needed per individual support plans (e.g., showering, toileting, assistance with eating)
- Ability to perform basic household tasks related to program operations (e.g., meal preparation, laundry, routine household cleaning, snow shoveling)
- Ability to operate or ride in a vehicle; driving is a requirement for most positions and includes driving agency vans, including wheelchair vans, or personal vehicles
- Ability to secure wheelchairs and assistive devices as needed.
- High school diploma or GED required
- At least one year’s experience and/or training in working with Individuals with intellectual, developmental disabilities and/or acquired brain injury.
- Valid driver’s licence and satisfactory driving record required.
- Basic computer skills for documentation and communication.
- First Aid, CPR, PABC certifications required.
