Job Type: RN or LPN (job share option for two positions is possible)

Schedule:
Initially 21 hour/week 3-7 pm or 4-8pm each day (Residential)

Schedule: 20-40 hours (Day Habilitation and Community Based Day Supports) Monday through Friday 8:30am to 3pm.

Location:
Our office is in Waltham with free parking.

Provide nursing and medical services to the individuals living in OFI's group homes. Responsibilities also include completion of progress notes, development of care plans, and other administrative duties. Full training is provided along with ongoing support. The focus of our group homes at this time is on the Turning 22 population.

Provide nursing and medical services to the individuals served (Day Habilitation and Community Based Day Supports) Monday through Friday 8:30am to 3pm. Responsibilities also include completion of progress notes, development of care plans, and other administrative duties. Full training is provided along with ongoing support. The focus of our Day Services ranges from medically involved to no nursing support needed.

• Provide nursing and medical services to clients
• Provide education regarding prescribed treatment
• Document in the progress notes the provision of care and the clinical and behavioral observations of clients
• Administer and document medications, adhering to state and program guidelines
• Provide first aid and emergency medical care as needed, following program guidelines
• Transcribe provider orders, following program protocols
• Measure and document client vital signs
• Other duties within the scope of practice as outlined by the Washington State Board of Nursing
• Attend all mandatory meetings
• Maintain licensure, certification, and other requirements for the position.
• Observe and assess clients in terms of therapeutic response to medication and the nursing care plan.
• Assist clients with activities of daily living as appropriate
• Assist in monitoring and documenting medical supplies, floor stock, and pharmacy communication.
• Assist in all aspects of the laboratory specimen, collection, and throughput.
• Adhere to program's infection control policy and protocols.
• Plan and lead educational and information groups for clients as assigned.
• Work to ensure effective communication among team members during and between shifts.
• Coordinate with other entities and staff regarding outside medical appointments and activities.
• Transport and attend medical appointments with individuals as needed.
• Work with team members to create and contribute to a positive learning environment for all.
• In addition to the above, perform other duties as assigned.

The nurse must be licensed by the MA Board of Registration, with at least two years of experience working in the field. Preference working with either elders or adults with disabilities. A valid driver's license and car are necessary. Be willing to become MAP certified and complete the mandated trainings by the Department of Developmental Services.
