Position: Occupational Therapist - Skilled Nursing ( Full Time or Part Time Available)

Occupational Therapist

The Occupational Therapist partners with residents and other care providers to provide occupational therapy services to maximize residents' functional abilities and independence in their activities of daily living.

Essential Duties and Responsibilities:

  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Performs evaluative and therapeutic procedures under the order of a physician or other accepted healthcare provider while observing precautions, indications, and contraindications.
  • Evaluates residents per department protocols and establishes a therapeutic plan of care that includes functional long- and short-term goals.
  • Provides ADL training, assisting residents with personal hygiene tasks such as bathing, dressing, oral care, and hair care.
  • Assists with ambulation, mobility, and positioning: transfers to and from bed/chair/toileting; includes the use of mechanical lifts as appropriate and proper body mechanics.
  • Seeks input from the resident and primary caregivers regarding the therapeutic plan of care.
  • Provides consultation to the referring physician and other members of the healthcare team regarding the therapeutic and discharge needs of the resident. Attends all pertinent team meetings.
  • Monitors and evaluates the treatment effect and progresses the treatment program toward the functional goals established in the treatment plan of care. Modifies the treatment plan of care as necessary.
  • Documents in the EMR all evaluative and therapeutic interactions with the resident per department procedures. Ensures that the documentation is accessible to others on the care team including the resident when appropriate by ensuring language that can be easily understood.
  • Recommends and obtains appropriate assistive devices to the resident ensuring that instruction has been given in their use.
  • Provides appropriate education to the resident or caregiver. Documents response to teaching.
  • Refers residents appropriately to Certified Occupational Therapist Assistants (if applicable). Works collaboratively to ensure good resident care with other therapy professionals.
  • Understands and complies with Medicare, Medicaid, and other reimbursement regulations. Submits charges for procedures per resident visit.
  • Supervises the activities of the assistant-level staff when appropriate, considering their level of education and training. Ensures adherence to all state mandates and professional organizations' standards for using assistant-level staff.
  • Provides mentorship to new staff during orientation, ensuring that the department orientation checklist and basic competencies are completed per departmental procedure.
  • Ensures department cleanliness and maintenance by routinely keeping office and clinical areas clean and neat. Assists with routine equipment and space cleaning. Follows all system and department infection control procedures.
  • Maintains required licenses, certifications, and competencies, and completes annual compliance courses on time.
  • Other duties as consistent with this role.

Education and Experience:

  • Bachelor's degree required. Masters or Doctorate preferred
  • Licensed as an OT in the State of PA
  • Basic Life Support (BLS) certification is preferred
  • Prior experience in a long-term care or skilled nursing facility preferred
