Position OverviewWe are seeking a Hospice Social Worker to provide emotional, psychosocial, and practical support to patients and families.
The Social Worker plays a key role in the interdisciplinary team and helps ensure patients receive holistic, patient-centered care.
Key Responsibilities- Conduct comprehensive psychosocial assessments
- Provide counseling and emotional support to patients and families
- Assist with advance directives and care planning
- Coordinate community resources and support services
- Participate in interdisciplinary team (IDT) meetings
- Maintain accurate and timely documentation
- Ensure compliance with hospice regulations and standards
- Licensed Social Worker in Massachusetts (LCSW or LMSW)
- Previous hospice or healthcare experience preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
