Physical Therapist

The Physical Therapist partners with residents and other care providers to provide physical therapy services. The goal is to maximize residents' functional abilities and independence in their activities of daily living.

Essential Duties and Responsibilities:

• Supports and promotes the mission and values of the organization.
• Performs evaluative and therapeutic procedures under the order of a physician or healthcare provider, following necessary precautions, indications, and contraindications.
• Evaluates residents according to department protocols and establishes a therapeutic plan of care.
• Seeks input from residents and primary caregivers regarding the therapeutic plan of care.
• Provides consultation to the referring physician and other members of the healthcare team about the plan of care and discharge needs.
• Attends all pertinent team meetings.
• Monitors and evaluates treatment effects and progresses the treatment program towards established functional goals.
• Modifies the treatment plan as necessary.
• Documents in the Electronic Medical Record all evaluative and therapeutic interactions as per department procedures.
• Recommends and obtains appropriate assistive devices for residents, ensuring adequate instruction in their use.
• Provides appropriate education to residents and/or caregivers and documents the results of teaching.
• Refers residents appropriately to the Physical Therapist Assistant as needed.
• Works collaboratively with other therapy professionals to ensure quality resident care.
• Understands and complies with Medicare and other reimbursement regulations.
• Provides in-service education regarding clinical knowledge and skills to nursing and other departments.
• Maintains current clinical skills and knowledge through ongoing learning activities.
• Assists with the development of new programs and services offered from Rehabilitation.
• Stays informed on professional issues through membership in organizations and reading professional publications.
• Brings pertinent matters to the attention of others on staff.
• Supervises the activities of assistant-level staff as appropriate, considering their education and training level.
• Ensures adherence to state mandates and professional organizations' standards for using assistant-level staff.
• Addresses performance issues with the Director as necessary.
• Provides mentorship to new staff during orientation.
• Ensures department cleanliness and maintenance by routinely keeping office and clinical areas clean and neat.
• Assists with the cleaning of equipment and space regularly.
• Follows all system and department infection control procedures.
• Maintains required licenses, certifications, and competencies, and completes annual compliance courses on time.
• Performs other duties as consistent with this role.

Education and Experience:

• Bachelor's degree required;
  Master's or Doctorate preferred.
• An equivalent combination of education and experience that provides proficiency in the areas of responsibility listed above may be considered.

Working Conditions/Physical Demands:

• Must possess the physical and mental abilities to perform tasks associated with this position. This involves a combination of sitting, standing, walking, bending, stooping, twisting, and reaching.
• Must be able to lift up to 50 pounds; lifting above 50 pounds requires a two-person lift. Lifting and carrying frequently involve up to 20 pounds.
• Some stress may be related to the high level of responsibility for quality care.
