- Develop and implement individualized patient care plans
- Perform in-home assessments and ensure quality care delivery
- Administer medications, IV infusions, and treatments as prescribed
- Provide advanced wound care, including Wound VAC (NPWT) management
- Maintain accurate documentation and ensure regulatory compliance
- Supervise and support nurses, aides, and caregivers
- Coordinate staffing and ensure proper coverage for patient cases
- Collaborate with physicians and healthcare professionals
- Active and unrestricted Registered Nurse (RN) license
- Current CPR Certification
- 2–3 years of clinical experience (home health, hospice, or skilled nursing preferred)
- Strong clinical, communication, and organizational skills
- Reliable transportation, valid driver’s license & auto insurance
- Experience with IV infusion therapy and complex care cases
- Wound care expertise (including NPWT/Wound VAC)
- Leadership or supervisory experience
- Ability to handle on‑call responsibilities and urgent situations
- Strong decision-making and problem-solving skills
- Passion for patient-centered care
- Ability to lead, mentor, and inspire a team
- Commitment to high standards and continuous improvement
