Position OverviewThe Hospice RN Case Manager is responsible for coordinating and delivering comprehensive nursing care to hospice patients.
This role supports patients and families throughout the end-of-life process.
Key Responsibilities- Perform nursing assessments
- Develop and manage care plans
- Administer medications and treatments
- Educate patients and families
- Coordinate interdisciplinary care
- Monitor patient condition
- Maintain accurate documentation
- Participate in on-call rotation
- Active RN license in Massachusetts
- 1–2 years nursing experience preferred
- Strong clinical and communication skills
- Ability to work independently
