Job Description - Human Services Coordinator I (260003PW)

The Department of Developmental Services (DDS) is excited to offer a career growth opportunity to a motivated and talented individual for the Human Services Coordinator I (HSC I) position at our Charles River West office in Waltham. The Human Service Coordinator I is the primary contact for individuals and families served by the Department and HSC I staff are expected to effectively represent the Area Office by building respectful and collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups involving the work of the Area Office and Department.

and Responsibilities

As a Human Services Coordinator I, you will coordinate and facilitate the development of Individual Service Plans (ISP) for individuals on your assigned caseload, and monitor the implementation of services provided by DDS funded residential, day and support program providers. You will ensure individuals receive appropriate services by arranging, organizing, and coordinating the Department of Developmental Services funded and generic support services in response to individual needs, while maintaining communication and outreach with community services and other state agencies.

Types
  • ASD SC:
    Autism Service Coordinator – supports Autistic adults who are DDS eligible and do not have a co‑occurring intellectual disability. You will facilitate service development with provider agencies and other resources in response to the individuals’ needs and desires. Collaboration with other State Agencies such as DMH, MRC, etc. is common.
  • Children’s SC:
    Children’s Service Coordinator – responsible for coordinating services for DDS‑eligible children (3‑22) and their families. Activities include assisting caregivers with planning, accessing generic community‑based supports, understanding SPED process, and referring to internal and external supports. Roles include service coordination, case management, information and referral, data management and inter‑agency connections.
  • Adult SC:
    Adult Service Coordinator – supports DDS eligible individuals and their families in identifying and accessing needed supports and the mechanisms by which those supports will be arranged. You will keep regular contact with individuals to ensure supports meet their needs and will visit service settings to meet the individual and observe their support.
  • Transition SC:
    Transition Service Coordinator – works with Chapter 688 (Turning 22 law) to provide a planning process for young adults with disabilities as they leave special education and transition into the adult service system. You will develop and maintain close working relationships with local school authorities and facilitate a stable transition for individuals from school to adult services.
  • Self‑Direction SC:
    Self‑Direction Service Coordinator – works with participants in the Participant Directed Program who are directing their own services. Responsibilities include serving as a support broker, helping the participant draft/revise a budget, supporting recruitment/interviewing/hiring of workers, and problem‑solving payroll and employer issues between participants and their employees.
Core Responsibilities
  • Monitor the safety and well‑being of assigned individuals.
  • Provide advocacy in human, civil, and legal rights.
  • Facilitate team meetings and monitor follow‑through on team recommendations while maintaining records and relevant databases.
  • Maintain regular and consistent contact with individuals, service providers, and families.
  • Arrange and/or provide crisis intervention as needed; assess needs and make recommendations for follow‑up planning.
Required Qualifications
  • At least three (3) years of full‑time or equivalent part‑time professional experience in human services work or social work, or an equivalent combination of experience and substitutions.
  • Substitution: A bachelor’s degree or higher in a related field may substitute for two (2) years of the required experience.
