Social Worker, Medical Social Worker

A skilled nursing facility caring for both the Pediatric and Geriatric populations. We are looking for a devoted and caring Social Worker for our Geriatric population to join our team!

The primary purpose of the position is to assist in the planning, organizing, developing, evaluating, and directing the overall process of the Geriatric social services department.

• Maintain and/or improve resident’s ability to control everyday physical, mental, and psychosocial needs

• Interview resident/families to identify the resident’s individual social and emotional needs

• Develop and revise written plans of care (preliminary and comprehensive) for each resident that identifies the needs of the resident and the goals to be accomplished for each need

• Participate in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

• Participate in discharge planning

• Refer resident to appropriate social service agencies when the facility does not provide for the services or needs of the resident

• Provide a positive image of the facility to all residents and family members

• Provide residents with information concerning resident rights, personal funds, advance directives, etc.

• Bachelor’s degree in social work

• Long Term Care/ Subacute Experience a plus

• Demonstrate competence in management and leadership skills

• Knowledge of state and federal regulations
