Join our dedicated team at Hospice Services of Massachusetts as a Hospice RN Case Manager in the Wareham area! As a valued member of our organization, you will play a crucial role in providing compassionate and high-quality care to patients with life-limiting illnesses and their families.

$42 to $52 an hour, depending on experience, with weekly pay.

Job Summary

We are seeking experienced and compassionate Registered Nurses to join our Hospice Case Management team. As an RN Hospice Case Manager, you will plan, organize, and direct hospice care, building from community resources to meet the needs of patients and families in their homes and communities.

Responsibilities
  • Supervise hospice aides
  • Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
  • Provide professional nursing care by utilizing all elements of the nursing process
  • Assess and evaluate patient status by writing and initiating a plan of care, regularly re-evaluating patient and family needs, and participating in revising the plan as necessary
  • Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
  • Administer medications and treatments as prescribed by the physician
  • Counsel patients and families in meeting nursing and related needs
  • Provide health care instructions to patients as appropriate
  • Assist patients with activities of daily living and facilitate self-sufficiency
  • Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseload
Requirements
  • Graduate of an accredited school of nursing
  • One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care
  • Current licensure in Massachusetts and CPR certification
  • Bachelor's degree, with one (1) year of hospice care experience preferred
