Ennoble Care is seeking an experienced Per Diem Nurse Practitioner to provide face-to-face assessments to hospice patients in the Gwinnett/DeKalb Counties, GA region. You will apply medical techniques under the supervision of a physician and care for a diverse set of patients with chronic diseases.
Collaborate with the hospice team to review eligibility, manage medications, educate families, document findings, and ensure timely F2F encounters within regulatory time frames.
