LPN​/Licensed Practical Nurse​/Facility Crisis Nurse

We are hiring for: LPN / Licensed Practical Nurse / Facility Based Crisis Nurse (Regular). Performs nursing functions and consumer care at a Facility Based Crisis Center, including assessment, care, and treatment of clients at the facility; must adhere to standards of care as defined by the Nurse Practice Act and RHA. This position relies on knowledge, experience, and judgment with limited supervision and direction to provide quality services to crisis facility clients. This position requires an active LPN license.

• Perform nursing assessments and interventions with medication oversight and administration as needed.
• Report client’s social, psychological and physical behavior as indicated.
• Implement and ensure compliance with standards of nursing practice that promote optimal health care delivery and meet regulatory requirements.
• Implement policies and procedures to ensure organizational compliance with confidentiality regulations, HIPAA, national accreditation agency and continuous quality improvement; attend applicable meetings as necessary.
• Use state guidelines, Quality Assurance Plan and Client Rights Policy to ensure clinical operations comply with state regulations and update plans and policies as necessary.
• Supervisory responsibilities include interviewing, making recommendations for hiring, training employees; planning, assigning and directing work; appraising performance; rewarding, motivating, disciplining and promoting team performance; addressing complaints and resolving problems.

• Active and current LPN licensure in the state of employment.
• Two years’ experience as a nurse working with behavioral health clients (preferred).
• Working knowledge of psychoactive substance abuse, dependency and principles and techniques based on best practices; commitment to evidence‑based treatment and preventative methods.
• Valid driver’s license and automotive insurance required if providing transportation for individuals.
• Pre‑employment screening: criminal background check, name check in relevant registries, drug testing, education verification and credentialing.
• Proof of employment history or references (if required).
• If driving is required: proof of driver’s license, driver’s insurance and vehicle.
