LPN Hilliard, OH Harmony Trace Assisted Living PRN - Part-time 6am-6pm

Harmony Trace located in Hilliard, Ohio is an assisted living community with a team of health care professionals that is unmatched in the area. We are owned and operated by National Church Residences, the nation’s largest provider of affordable senior housing and health care services. We are seeking a LPN Charge Nurse who will share in our vision to advance better living and care for seniors.

Position Overview

The LPN Charge Nurse makes observations on an advanced level, reports and records observations appropriately, forms nursing judgments and carries out nursing actions based on an understanding of scientific principles.

Responsibilities
  • Supervise, provide, and direct nursing care in compliance with facility policies and procedures. Make advanced observations, report and record them, form judgments and act accordingly. Follow established standards of nursing practices; assume responsibility for compliance with federal, state, and local regulations; interpret existing policies for staff, families, and physicians. Perform daily rounds, develop and implement accurate comprehensive care plans. Follow policies for medication passing, order and credit medications, keep accurate accounting of all medications (stock, unit dose, narcotics).

    Follow staffing guidelines and make nursing assignments. Receive and give accurate reports on residents’ condition. Monitor diets, supervise meal trays and snacks, assist in passing meal trays, feeding, and snacks. Follow admissions procedures and communicate admissions to other departments and providers.
  • Supervision. Ensure employees follow chain of command per personnel policies. Assign nursing assistants and ensure efficient completion of assignments. Supervise, document, and correct poor performance; keep Unit Managers/Director of Nursing informed of problems. Participate in performance evaluation of nursing assistants and provide counseling, coaching, and training. Present performance evaluations.
  • Communicate professionally and effectively: follow written and verbal directions; provide exemplary customer service; report changes in physical or mental conditions promptly; follow all company, facility, and state policies; attend staff meetings, in-services, multidisciplinary team and committee meetings as assigned; exercise keen observations of noise, odors, cleanliness; cooperate with other staff to ensure quality environmental conditions.
  • Document resident information timely, accurately, and confidentially according to procedures. Document changes and notifications to POA and doctor; perform comprehensive assessments; verify change‑over MARS/TARS and update new MAR/TAR by the 1st of each month.
  • Adhere to guidelines in Resident Rights documentation: know resident rights; report complaints; maintain confidentiality.
  • Adhere to Safety & Compliance policies: ensure residents are safe; demonstrate proper use of equipment; report equipment needs; use protective equipment, follow infection control protocols; practice universal precautions and life safety measures.
  • Qualifications
    • Education:

      Graduate of an accredited nursing school or vocational practical nursing school.
    • Experience:

      One to two years preferred.
    • Good communication, comprehension, and interpersonal skills.
    • Working knowledge of Windows, Microsoft Office Suite, and the Internet.
    • Current nursing license.
