Provide comprehensive HIV case management services, including support, care coordination, and treatment for individuals living with HIV/AIDS and their families. Conduct client assessments, coordinate medical referrals, facilitate access to social support services, develop individualized care plans, and assist clients with medication access. Operate in a fast‑paced clinical environment, providing nursing support and laboratory coverage as needed.

• Associate's degree in nursing from an accredited college or university.
• Completion of a PHN course within one year of hire.
• Current Georgia Registered Professional Nurse license.
• CPR certification.

• Bachelor’s degree in nursing.
• At least one year of experience as a registered professional nurse.
• Experience working with HIV/AIDS patients.

SEHD encourages staff to incorporate wellness activities and supports up to 30 minutes of wellness breaks per workday.

Current state employees are subject to State Personnel Board rules regarding salary. Candidates may be required for pre‑employment drug screening, criminal background check, and fingerprint investigation.
