Position: RN Navigator - Interventional Radiology (full time)

Work Shift

Day - 8 Hour (United States of America) FT, 8:00 a.m. – 4:30 p.m.

Job Summary

Performs and oversees transitional care management processes for recently discharged patients of HHC, providing the best possible care options for patients and their families. Works directly with Care Management and HHC Independent physicians to collaborate, monitor, and evaluate patient status post‑discharge focusing on intermediate goals, outcomes, medication reconciliations and prevention of readmissions. Ensures smooth hand‑offs from hospital to home and coordinates appropriate after‑care activities such as paramedicine, home health, DME, rehab services, educare, pharmacy, and transportation.

Responsibilities
  • Collaborate with physicians and interdisciplinary teams to triage, coordinate, and manage patient care.
  • Act as primary point of contact for patients and families, answering questions, providing emotional support, and solving care cycle issues.
  • Serve as a resource for staff and independent physicians regarding patient care after discharge.
  • Ensure accurate medication reconciliation and clear communication to prevent confusion.
  • Coordinate and monitor after‑care activities to meet patient needs.
Qualifications
  • Education: Graduate of an accredited school of nursing; BSN required.
  • Experience: Minimum two years of nursing experience; preferred one year of case management experience.
  • Certification:
    • Basic Life Support (BLS)
    • Active compact/multistate license (eNLC) required within 60 days of hire if virtual care activities include multi‑state practice; employees with prior role must obtain compact status before next renewal.
Physical Requirements
  • Lift up to 50 lbs; carry objects up to 25 lbs.
  • Occasional to frequent standing, walking; occasional sitting; close eye work (computers, typing, reading, writing).
  • Physical demands may vary depending on assigned work area and tasks.
Environmental Factors
  • Exposure to blood‑borne pathogens, biohazardous waste, chemicals, gases, fumes, vapors, communicable diseases, electrical shock.
  • Environmental conditions may vary; requires use of personal protective equipment.
  • Shift work and travel may be required.
