The Licensed Practical Nurse (LPN) provides direct patient care and clinical support under the supervision of a Registered Nurse, physician, or other licensed provider. This role is integral to the clinical team, assisting with patient assessments, treatment preparation, medication administration, and documentation to ensure a safe, efficient, and compassionate care experience for all patients.

ESSENTIAL DUTIES & RESPONSIBILITIES Patient Care
  • Provide direct patient care appropriate to the patient's age, condition, and treatment plan within scope of LPN licensure.
  • Take and document patient vital signs, medical histories, and chief complaints accurately and in a timely manner.
  • Prepare patients for examinations, procedures, and treatments.
  • Administer medications and treatments as ordered by the physician and within the LPN scope of practice.
  • Assist physicians and RNs during clinical procedures and examinations.
  • Monitor patient responses to treatment and report changes in condition to the supervising provider.
Documentation & Compliance
  • Accurately document patient care, vital signs, medication administration, and clinical observations in the EMR system.
  • Maintain compliance with HIPAA, OSHA, infection control standards, and all applicable regulatory requirements.
  • Maintain required clinic logs including medication, safety, and temperature records.
  • Adhere to Partner Care policies, procedures, and clinical protocols at all times.
Collaboration & Communication
  • Communicate effectively with physicians, RNs, medical assistants, and administrative staff to support coordinated patient care.
