The Registered Nurse provides comprehensive skilled nursing care to patients in their homes in accordance with physician orders, the established plan of care, and regulatory requirements. The RN is responsible for patient assessments, care planning, coordination of services, supervision of LPNs and aides as applicable, and ongoing evaluation of patient outcomes. This role requires independent clinical judgement, consistent travel within the service area, and accountability for regulatory compliance, documentation accuracy, and patient safety.

• Conduct comprehensive initial, ongoing, and recertification patient assessments
• Develop, implement, review, and update individualized plans of care in collaboration with providers and the interdisciplinary team
• Provide skilled nursing services including but not limited to wound care, complex medication management, IV therapy as applicable, injections, catheter care, enteral feedings, chronic disease management, and symptom management
• Evaluate patient responses to treatment and modify care plans as clinically indicated
• Supervise, direct, and provide clinical oversight to LPNs and home health aides as applicable
• Communicate timely and significant changes in patient condition to the provider and Home Health Director
• Coordinate care with physicians, therapists, social workers, pharmacies, and other healthcare providers
• Complete accurate, timely, and compliant documentation in Meditech EMR, including assessments, visit notes, care plans, OASIS data if applicable, medication reconciliation, and patient education
• Ensure compliance with CMS Home Health Conditions of Participation and all applicable state and federal regulations
• Provide patient and caregiver education related to disease processes, medications, treatments, safety, and self-care
• Participate in case conferences, interdisciplinary meetings, quality improvement activities, and required education
• Monitor supply utilization and ensure appropriate use of resources
• Identify home safety concerns and implement or recommend interventions to reduce risk
• Participate in on-call rotation as assigned
• Maintain patient confidentiality in accordance with HIPAA and organizational policy
• Comply with all BCMC policies, procedures, and professional nursing standards
• Perform other duties as assigned

• Active Arkansas Registered Nurse license in good standing
• Minimum of one year RN clinical experience required; home health experience preferred
• Valid driver’s license and reliable transportation
• Ability to travel throughout the BCMC Home Health service area

• Strong clinical judgement and critical thinking skills
• Ability to independently assess, plan, and manage patient care in the home setting
• Knowledge of home health regulations, documentation standards, and care coordination requirements
• Strong leadership and supervisory skills
• Effective communication skills with patients, families, providers, and interdisciplinary team members
• Proficiency with EMR systems, including Meditech, or willingness to learn
• Ability to prioritize workload, manage time effectively, and meet documentation deadlines
• Professional demeanor and accountability when working independently
• Home-based patient care with frequent travel
• Exposure to varying home conditions, pets, allergens, and environmental risks
• Regular use of BCMC-provided medical equipment and supplies
• Occasional on-call and after-hours responsibilities

• Ability to stand, walk, bend, lift, and transfer patients
• Ability to carry medical supplies and equipment to and from patient homes
• Ability to work extended or irregular hours as required to meet patient care needs
