A social worker is responsible for ensuring that the medically related emotional and social needs of the patients are met and maintained on an individual basis in accordance with policies and procedures, and current TJC, federal, state, and local standards, guidelines, and regulations.

• Performs social service admissions functions including the development of comprehensive social history and psychosocial assessments.
• Participates in the development of individualized patient care plans involving the patients and their families in care planning.
• Attends scheduled care conferences and identifies and interprets psychosocial needs of patients for inclusion in the care plan, addressing issues of the care schedule, treatment, and environment that express and reinforce individuality and identity.
• Plans discharges/transfers to ensure appropriateness and continuity of care.

• Minimum of a Master's in Social Work with an LLMSW. LMSW preferred.
• Currently licensed as a Social Worker in the State of Michigan
• Three years (3) experience in a clinical setting or as a licensed counselor preferred
• Demonstrated competency in diagnostic developmental assessments.
• Demonstrated ability to maintain and manage a caseload, complete records in a timely manner, to provide a variety of services, and to work harmoniously with professional and clerical personnel.
