You Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team. Help patients regain strength, mobility, and independence while using modern technology and AI-supported tools to simplify your workload and support a better work-life balance.

• Evaluate patients and develop individualized physical therapy plans of care
• Provide physical therapy treatments to improve mobility, strength, balance, and coordination
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on therapeutic exercises and safety techniques
• Complete accurate and timely documentation
• Use AI-supported tools to streamline documentation and daily workflows
• Travel to patient homes within the assigned service area
• Provide compassionate, patient-centered care

• Licensed as a Physical Therapist in the state of employment

• Licensed or registered in the State of employment
• Must pass a criminal background check & MVR check.
• Completed health statement acknowledging ability to perform the duties of the position.
• Valid state driver's license and provide proof of current automobile insurance.
• Current CPR card;
  On-line CPR certification accepted;
  Hands-on CPR certification preferred.
• TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
