Reports to: Medical Director
Job SummaryUnder the direction of the Medical Director, the FNP shall serve as a team member in the provision of comprehensive health services in accordance with the State of Michigan Nursing Board guidelines. The FNP will provide primary healthcare services to patients across all lifespans, focusing on preventative care, diagnosis, and treatment of acute and chronic illnesses.
Primary Duties and Responsibilities- Works collaboratively with multidisciplinary teams, including social workers, therapists, and case managers, to assure optimal support and care to all patients while assuring cost effective use of all treatments including medications.
- Conducts primary care evaluations, assessments, and diagnosis for clients.
- Develop and implement individualized treatment plans.
- Works collaboratively with primary care providers to assure prescriptions are not duplicated, appropriate lab tests are ordered and screened with follow up.
- Adheres to the practice standards of the local community and State of Michigan Nursing Board regulations and policies related to the treatment of persons suffering from substance abuse and mental illnesses.
- Provides education to patients, their families, and to staff members related to medication, diagnosis and medical conditions.
- Adheres to guidelines related to pharmacy practices.
- Responsible for meeting encounter requirements, as specified.
- Records and documents patients’ information in a quality and timely manner.
- Provides all patients with appropriate referrals to other community agencies as needed or requested.
- Follows professional and ethical practices when working with patients, patient records patients’ families, and community personnel.
- Maintains confidentiality of patient/family information.
- Participates in required trainings.
- Attend all scheduled team meetings.
- Completes all other assignments or duties as designated.
- Licensed as an FNP by the Michigan Board of Nursing.
- Master’s or Doctorate degree in nursing from an accredited program.
- Licensed by the Drug Enforcement Administration.
- Minimum of 2 years of clinical experience in a primary care, urgent care, or medical clinic setting preferred.
- Valid Michigan Driver’s License and Insurance
- Must be a practitioner in good standing with the agency with practice and privileges consistent with clinical qualifications.
- Experience working in community-based setting preferred. Strong diagnostic, problem-solving, and communication skills.
- Commitment to patient-centered care and community well-being.
- Competent computer skills using EMR, electronic prescriptions, and MS Office products.
- Ability to provide culturally competent services that shows sensitivity to the service population’s cultural and socioeconomic characteristics.
