As a Full-Time MDS Coordinator you play a critical role in ensuring accurate clinical documentation, regulatory compliance, and appropriate reimbursement—while supporting high-quality, resident-centered care.

What You’ll Do- Coordinate and complete MDS assessments from start to finish, making sure everything is accurate and submitted on time
- Manage daily and weekly MDS schedules (ARDs), keeping track of due dates and making sure nothing falls behind
- Review resident charts, therapy notes, and nursing documentation to ensure assessments reflect the full clinical picture
- Work closely with nursing, therapy, dietary, and social services to gather the right information and keep documentation aligned
- Lead or participate in care plan meetings, helping ensure each resident’s plan reflects their needs and goals
- Identify missing or inconsistent documentation and follow up with the team to resolve it before submission
- Monitor Case Mix (CMI) and support accurate reimbursement by ensuring documentation supports the services being provided
- Assist with audits, surveys, and quality reviews by maintaining organized, compliant, and survey-ready documentation
- Help educate and support staff on documentation best practices to improve accuracy and outcomes
- Serve as a key point of contact for anything related to MDS, care planning, and regulatory compliance
- Current, active Registered Nurse (RN) state license OR LPN actively enrolled in an RN program
- Long-term care experience preferred
- Prior MDS Coordinator experience preferred, but training is available for the right candidate
- Strong written and verbal communication skills
- Detail-oriented, organized, and deadline-driven
