Description

The Nurse Practitioner (NP) plays a vital role in delivering compassionate, high-quality care to participants of Huron Valley PACE. As a key member of the interdisciplinary care team, the NP provides comprehensive assessments, diagnoses, and treatment plans that support the health, independence, and well-being of older adults. Working closely with participants, families, and healthcare professionals, the NP helps coordinate personalized, patient-centered care while ensuring clinical excellence, regulatory compliance, and positive health outcomes under the guidance of the Medical Director.

Key Responsibilities Patient Care & Clinical Responsibilities
  • Perform health histories, physical exams, patient education, selected procedures, and documentation.
  • Order, interpret, and follow-up on lab tests and results.
  • Assess, initiate, and coordinate patient care daily under the supervision of the attending physician.
  • Recognize abnormalities that require immediate referral for further intervention.
  • Initiate and evaluate orders for medications, diagnostic studies, and treatments within physician-approved protocols.
  • Provide emergency care as needed and initiate emergency measures when indicated.
  • Collaborate with ER physicians and hospital staff to manage care, prevent unnecessary hospitalizations, and facilitate expeditious discharge.
Collaboration & Care Coordination
  • Act as a liaison with physicians, nurses, and other healthcare professionals to ensure integrated care.
  • Collaborate with attending physicians to request consultations from contracted services/providers as needed.
  • Coordinate care with outside providers, including nursing homes, assisted living, adult foster care, and hospitals.
  • Participate in interdisciplinary team (IDT) meetings, contributing to holistic patient care planning.
Patient & Family Education
  • Educate participants and families on preventive health maintenance, particularly for chronic conditions.
  • Actively participate in family conferences to provide education and care planning.
Quality Improvement & Compliance
  • Actively engage in quality improvement (QI) processes to enhance care delivery.
  • Represent Huron Valley PACE in interactions with external agencies, professional groups, and regulatory bodies.
  • Consistently act within the scope of practice as defined by the State of Michigan.
Documentation & Reporting
  • Document participant changes accurately in the medical record.
  • Communicate significant changes to team members to ensure coordinated care.

Skills, Knowledge & Expertise
  • RN licensure in the State of Michigan and certification from a Nurse Practitioner Program
  • Bachelor’s Degree in Nursing from an accredited college or university, with graduation from a certificate or accredited Nurse Practitioner program.
  • 5–7 years of nursing experience, with a minimum of 3 years as a Nurse Practitioner, or a Master’s Degree in Clinical Nursing and 5 years of nursing experience.
  • Experience working in a PACE (Program of All-Inclusive Care for the Elderly) facility.
  • Experience in a managed care environment with 2–3 years of nursing experience specifically with geriatric patients.
  • Proof of clearance for all communicable diseases prior to participant contact.
