Job Summary
Freedom at Home seeks a Home Health Registered Nurse (RN) to provide skilled nursing services in Oakland and Macomb counties. The role may be part-time, per diem, or full-time.
A professional who provides nursing services within scope of practice standards in collaboration with primary care physicians, teaches and educates patients and families. Professional skilled nursing is performed under the supervision of the Administrator/Director of Nursing.
Qualifications- Graduate of an approved professional nursing program (RN)
- Licensed "in good standing" in MI
- Minimum 1 year of professional nursing experience, preferably in home care
- Current CPR certification
- Valid criminal background check
- Provide nursing services requiring substantial and specialized nursing skill; initiate appropriate preventative and rehabilitation nursing procedures.
- Prepare and submit timely clinical and progress notes; oversee coordination of patient services.
- Inform MD and staff of changes in patient status; complete medical history, home safety check, and environmental assessments; perform socio-psychological evaluation.
- Perform assessment visits and document timely (OASIS, Skilled Nursing Note, etc.); perform physical examination and review all body systems.
- Develop individualized Plan of Care (POC), submit to physician for approval, and perform necessary revisions based on patient needs.
- Determine medical necessity for other services; regularly re-evaluate the patient’s nursing needs.
- Evaluate the patient’s ADL and iADL abilities and need for home health aide; develop and implement HHA plan of care when ordered; revise and sign care plan for each certification period.
- Supervise HHA in accordance with state/federal requirements and document supervision; order other professional services as appropriate.
- Review billing processes with patient/family, advising when co-pay or Medicare may not cover services.
- Communicate on progress, changes in POC, and coordination with supervisor, nursing director, and other disciplines.
- Communicate with patient’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities.
- Participate in case conferencing sessions to establish best practices; submit requests for re-authorization of additional visits to payers in a timely manner.
- Coordinate community services for patient to assist in safe home care needs; participate in the performance improvement committee.
- Perform timely clinical record reviews (CRR) per agency policy; participate in ongoing staff meetings/in-services; serve as a preceptor for same-discipline employees.
- Notify agency of emergencies, sickness, and other occurrences that may affect patient caseload quickly.
- Submit written time requests at least 2 weeks in advance of planned time off; perform other duties as assigned.
Vehicle for routine patient visits; possible transfer/lifting devices; medical supplies.
Work Environment- Indoors, performing patient care assessment/tasks in patient residences.
- Travel to patient residences/meetings; requires transportation to various work sites; possible exposure to communicable diseases.
Able to access patient’s homes that may not be handicap accessible; hear, speak, understand, and communicate effectively in English.
Physical RequirementsWalking, standing, sitting, reaching, use of telephone, use of computers and other office equipment; lifting/transferring 150 lbs or greater; bending, stooping, climbing stairs.
