Community Skilled Health Care Centre is a trusted skilled nursing and rehabilitation facility located in Warren, Ohio. We pride ourselves on providing quality, patient-centered care in a supportive, team-focused environment.
Position Summary:We are seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary rehab team. As a PT, you will help residents regain strength, mobility, and independence so they can enjoy the best quality of life possible.
Responsibilities:- Evaluate patients and develop individualized treatment plans.
- Provide direct therapy services in accordance with physician orders.
- Work collaboratively with nursing, occupational therapy, and speech therapy teams.
- Educate patients, families, and caregivers on treatment plans and safe home exercises.
- Accurately document treatments and progress.
- Current Ohio Physical Therapist license in good standing.
- Experience in a skilled nursing facility or rehab setting preferred, but new grads are welcome to apply!
- Strong communication and teamwork skills.
- Commitment to delivering excellent patient care.
