About the Role:As a Social Worker/Care Companion, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. This is an ideal opportunity for professionals who want to make a direct impact on the lives of older adults and help them feel heard, supported, and emotionally connected within their environment.
Key Responsibilities:- Visit designated facilities in accordance with the schedule set with your supervisor
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
- Document visit notes and maintain detailed records of interactions and interventions after each visit
- Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
- Associate degree in Social Work, Psychology, Counseling, or another mental health-related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- 1 year of experience
- Meaningful, resident-focused work in a supportive team environment
- Regular supervision and collaboration with psychiatric providers

- Flat salary: $8,000 - $10,000 annually
- Pick the day and hours that work for you! Approximately 7 hours per week during Monday-Friday, within regular 8-5 hours
