Position: Licensed Practical Nurse (LPN)
Hours: Averages 30 hours weekly (12‑hour shifts, 8:00 am–8:00 pm)
Summary of Primary FunctionUnder the direction of a Physician or Nurse Practitioner, the LPN delivers patient care using the nursing process, including data collection, assessment, prioritization of patient needs, and evaluation of nursing outcomes. The LPN coordinates teaching activities and collaborates with the health team while adhering to documentation standards.
Essential Job Functions- Analyze assessment data, evaluate patient condition, and collaborate with the provider to develop and maintain the patient’s plan of care.
- Provide education to patients, families, and staff in conjunction with other healthcare entities, under the direction of the provider or manager.
- Coordinate patient care with other healthcare entities according to the patient’s plan of care.
- Administer medications safely in accordance with organizational policies, procedures, and applicable state requirements.
- When in a lead position, serve as a subject‑matter expert and clinical mentor within the practice.
- Current Licensed Practical Nurse (LPN) from the state of employment (required).
- BLS certification preferred upon hire and required within 30 days and/or prior to independent patient care.

Work Experience1 year of clinical patient care experience preferred.
TrainingEPIC Electronic Health Record (EHR) training (preferred).
