Job Summary:

We are seeking compassionate and skilled Floor Nurse professionals to join our team at Washington Square Nursing & Rehab, an 83-bed skilled nursing facility located in Warren, OH. As a Floor Nurse, you will play a vital role in providing high-quality patient care to our residents under the direct supervision of our Director of Nursing. • Provide direct patient care to our residents on the floor, including administering medications, wound care, and other treatments as needed
• Develop and implement individualized care plans in collaboration with our interdisciplinary team
• Monitor and report changes in patient condition to the Director of Nursing and other relevant staff members
• Maintain accurate and up-to-date records and reports
• Participate in quality improvement initiatives and patient satisfaction surveys
• Collaborate with other healthcare professionals to ensure continuity of care

• Current LPN or RN licensure in the State of Ohio
• 0 years of experience (new grads welcome to apply)
• Ability to work varying shifts, including days, evenings, and weekends
• Strong communication and interpersonal skills
• Compassionate and patient-focused approach to care

• Hourly rate of $26.39 - $37 based on certification and years of experience
