Aboutthe Social Services Director Role
As our Social Services Director, you will be responsible for overseeing the social services department, providing leadership and guidance to our social work team, and developing and implementing social services programs to meet the needs of our patients. You will work closely with our interdisciplinary team to ensure seamless communication and collaboration to achieve optimal patient outcomes. Your expertise and professionalism will enable you to build strong relationships with our patients, families, and caregivers, providing emotional support and counseling to navigate the rehabilitation process.
SocialServices Director Responsibilities
- Provide leadership and strategic direction to the social services department
- Develop and implement social services programs to meet the needs of patients, including case management, counseling, and community resources
- Supervise and mentor social work staff, providing coaching and guidance to enhance skills and performance
- Collaborate with healthcare professionals to develop and implement comprehensive treatment plans that address patients' physical, emotional, and social needs
- Conduct patient assessments and develop individualized care plans to ensure patient-centered care
- Develop and maintain relationships with community organizations and resources to access additional support services for patients
- Manage and maintain accurate records and documentation of patient services and outcomes
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Minimum of 2 years of experience in social services or a related field
- Social Service Director experience preferred
- A degree in Social Work, Psychology, or a related field is a plus
- Strong leadership and communication skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Excellent problem-solving and conflict resolution skills
